일본 개호보험 5기 개혁동향과 시사점

국문 초록

일본의 주요개정현황이 한국 장기요양보험제도에 시사하는 점을 실무자 관점에서 고찰해보는 것이 본 연구의 주요목적이다. 일본 개호보험의 5기 개정의 핵심은 개호서비스의 질을 확보하는 내용이 주를 이루고 있다. 일본의 주요개정을 3가지로 함축하자면 첫째, 일본의 지역포괄케어시스템을 개호보험의 기반으로 설정하고 이용자의 욕구에 유연하게 대응하기 위해 신규급여를 창설 및 지역포괄지원센터의 기능을 강화하고 있다. 둘째, 개호인력의 확보와 질을 개선하고자 사업자에 대한 노동법규준수를 강제화하는 조치와 정보공표제도에 서비스의 질과 종사자에 관한사항을 새로 신설하였다. 마지막으로 중앙정부와 지방정부에 대한 명확한 책임을 규정함과 동시에 지방정부의 권한강화를 위한 개정을 진행하고 있다. 일본의 5기 개정은 2005년 3기개정의 문제점을 보완하기 위한 개정이므로 5년 단위로 기본계획을 수립하여야 하는 한국의 장기요양보험정책에 시사 하는 점이 크다고 할 것이다. 따라서 한국의 장기요양보험의 기본계획의 틀은 지역사회 중심의 서비스제공체계 수립과 장기요양서비스 질 및 인력확보, 지방정부의 책임과 권한을 강화시키는 방안이 필요하다.

영문 초록

According to the 5th reform of the Japanese long-term care insurance, we could find that it focused on improving the quality of the service benefits To sum up, the first thing was that LCCS(Local Comprehensive Care System) of Japan became fundamental to its long-term care insurance. In addition, to meet a variety of benefiters' demands, the coverage of its benefits was enhanced and the role of LCCS was reinforced. Secondly, to secure the stable human resources and improve their quality, the measures that the employers had obligations to observe the labor legistration were taken and new regulations regarding service quality and employees were added to the existing "Public Information Disclosure System" Lastly, its reform included regulating the definite responsibilities which the central and local government will take as well as the enhanced authority which the local government will have. These reforms were established to supplement the weakness of its 3th reform in 2005. So, in my point of view, it will exert a strong influence on the policies of the Korean long-term insurance which its government established or renewed on the basis of every five years. Therefore, from the perspective of practicing professional, I will conduct researches regarding the influence of Japanese long-term care insurance reform on Korean one.
