국문 초록

목 적： 본 연구는 지역사회 거주노인의 치매선별을 위한 한국판 간이정신상태검사(Korean version of Mini-Mental State Examination：MMSE-K)의 적정 절단점수를 산출하고, 노인의 연령, 성별, 교육수준, 거주지역, 그리고 우울증의 유무에 따라 그 절단점수가 달라지는지 조사하고자 하였다. 방 법： 광주광역시 두 개 지역에 거주하는 65세 이상의 노인들에 대해 인지기능(MMSE-K)과 인구학적 특징(연령, 성별, 교육수준, 거주지역)이 조사되었다. 이들 중 MMSEK 24점 이하인 노인들을 대상으로 임상적 치매진단(DSM-Ⅳ)과 우울증(Hamilton Rating Scale for Depression)이 평가되었다. 치매진단에 대한 MMSE-K의 적정 절단점수와 그에 따른 민감도 및 특이도는 receiver operating characteristics curve 분석을 통해 파악되었다. MMSE-K 절단점수와 인구학적 특성 및 우울증과의 연관성은 오즈비로 추정되었다. 치매진단과 무관하게 MMSE-K 절단점수와 유의한 연관성을 보인 인자에 대해서는 각 수준 별로 적정한 절단점수가 재측정되었다. 결 과： MMSE-K 24점 이하인 노인들(n=341)에서, DSM-Ⅳ 치매진단에 대한 MMSE-K의 적정 절단점수는 21/22점이었으며, 이 점수에 따른 민감도 및 특이도는 각각 81%와 64%였다. 조사된 특성들 중 연령만이 MMSEK 절단점수와 연관성이 있었고, 75세 이상 고령군에서는 1점 낮은 20/21점이 적정 절단점수로 측정되었다. 결 론： 본 연구에서 조사된 치매선별을 위한 MMSE-K 절단점수가, 지역사회 보건 의료 종사자들에게 인지장애가 의심되는 노인을 평가하거나 전문가에게 의뢰를 고려할 수 있는 지침으로서 이용되기를 제안한다.

영문 초록

Objectives：This study aimed to estimate the optimal cut-off score on Korean version of Mini-Mental State Examination (MMSE-K) for screening of dementia in community dwelling old people, and to investigate possible changes of the cut-off score according to age, gender, educational level, living area, and depression of the old people. Methods：For the old people aged 65 or over dwelling in two areas of Kwangju, cognitive function (MMSE-K) and demographic characteristics (age, gender, educational level, living area) were investigated. For those scoring 24 or less on MMSE-K of them, clinical diagnosis of dementia (DSM-Ⅳ) and depression (Hamilton Rating Scale for Depression) were assessed. The optimal cut-off score on MMSE-K with a sensitivity and a specificity was examined by using receiver operating characteristics curve analysis served by diagnosis of dementia as a gold standard. Associations of the cut-off score on MMSE-K with demographic characteristics and depression were estimated by odds ratios. For those factors showing significant association with cutoff score on MMSE-K regardless of the diagnosis of dementia, the optimal cut-off scores on MMSE-K were measured again at each level. Results：In the old people scoring 24 or less on MMSE-K (n=341), the optimal cut-off score on MMSE-K served by diagnosis of dementia was 21/22 with a sensitivity of 81% and a specificity of 64%. Only the age among the investigated characteristics was significantly associated with the cut-off score on MMSE-K, and the optimal cut-off score in those aged 75 or over was revealed to be 20/21. Conclusions：We suggest that the cut-off score on MMSE-K for screening of dementia inves-tigated in this study could be used by community health care staff as a guideline for assessing old people with questionable cognitive dysfunction or for considering to consult them to an expert.
