국문 초록

불쾌역치를 측정하는 목적은 보청기 선택 및 평가에서 역동범위의 상위한도를 구하는 것으로 이는 보청기의 최대출력을 결정하는 중요한 요소가 되며, 진단 목적으로는 난청의 유형을 판단하는데 도움이 된다. 본 연구에서는 정상청력을 가진 20대 성인 43명(남자18명, 여자25명)을 대상으로 지시문을 이용하여 순음 불쾌역치의 검사-재검사 신뢰도를 측정하였다. 측정 주파수는 500, 1000, 2000, 4000Hz 영역에서 실시하였으며, 지시문은 McCandless(1973)가 제시한 것을 근거로 작성하였다. 피검자에게 소리에 대한 인식 및 친숙화를 위해서 연습시행을 한 후 추가로 3회 실시한 값의 평균을 불쾌역치로 정의하였다. 연구결과, 첫째, 주파수평균순음 UCL의 검사-재검사 신뢰도가 모두 높게 나타났다. 둘째, 주파수별순음 UCL의 검사-재검사 신뢰도가 모든 검사주파수에서 높게 나타났다. 셋째, 주파수별 및 주파수평균순음 UCL이 검사-재검사 모두에서 측정횟수에 따른 변화가 거의 없었다. 따라서 명확한 지시문과 검사절차를 사용한다면 1회 시행만으로도 신뢰도 있는 UCL을 얻을 수 있다고 본다. 하지만 대상자의 연령과 청력정도를 고려한 추가자료가 구해져야 임상에서 보다 효과적으로 적용할 수 있을 것이다.

영문 초록

This study investigated the test-retest reliability of pure tone UCLs. The UCLs were obtained from 43 normal-hearing subjects all of whom had normal otoscopy, middle ear pressure within +/-50daPa. This study integrated the methodologies of previous studies in terms of test stimuli, instructional set, and psychophysical method. UCL measurements were obtained from pure tones of four frequencies(500Hz, 1000Hz, 2000Hz and 4000Hz) using a simple ascending method. The listener’s task was to judge whether the stimu- lus was uncomfortably loud using a rating scale of bounded loudness categories. UCL measurements were repeated twice within a period of 2 weeks. The results of the study could be summarized as follows: (1) the test-retest reliability was high in all test con- ditions, (2) the average UCLs for the pure tones was 77.23dBHL in the test and 76.94dBHL in the retest, (3) UCLs were higher in male subjects than in female subjects, (4) there were no significant differences in the UCLs among the three sessions. In addition, no practice effect was found. One precondition that we noticed was that the subjects of the experiment should be well aware of the aim of the test and the procedures thereof. These results suggest that if subjects clearly understand the purpose and procedures of UCLs measurements, reliable data can be obtained even with a single trial of measurement.
