제II급 전치부 개방교합 환자의 골격성 고정원을 이용한 치험례

국문 초록

개방교합의 원인은 유전적 원인과 환경적 원인으로 구분할 수 있다. 유전적으로는 선천적 성장 잠재력이 수직적 방향으로 발현되어 발생하고, 환경적으로는 습관이나 하악 올림근의 위축, 구치부 과맹출, 하악 과두의 문제 등에 의해 발생할 수 있다. 개방교합은 골격성과 치성으로 그 특성으로 구분할 수 있는데, 골격성 개방교합은 주로 후방 구치부위의 과도한 수직성장이나 짧은 하악지의 길이로 인해 발생하고, 치성 개방교합은 주로 손가락 빨기 등의 습관에 의해 발생한다. 전치부 개방교합을 동반한 골격성 제II급 부정교합은 최근 골격성 고정원의 사용으로 악교정 수술 없이도 주소 해결이 가능하다. 특히 전안모 고경이 크고 hyperdivergent한 환자에서 상악 전치 노출도가 양호하고 구치부와 2개의 교합 평면을 이루는 경우, 구치부 압하를 통해 전안모 고경의 개선뿐 아니라 후퇴된 하악이 autorotation 되어 전치부 수직 피개를 증가시킬 수 있다. 수직적 관계의 변화는 악골의 전후방 관계에도 영향을 미치므로 입술 심미에 대한 치료 계획을 반드시 고려해야 한다. 치아 이동량이 적은 경우에는 miniscrew를 사용하나 치아 이동량이 많아 골격성 고정원의 탈락이 우려되는 경우에는 miniplate를 사용하는 것이 유리하다. 본 증례에서는 전치부 개방교합을 동반한 hyperdivergent한 안모의 골격성 제II급 부정교합 환자에서 골격성 고정원을 사용하여 구치부 압하와 전체 치열의 후방이동이 성공적으로 이루어졌고 양호한 치료 결과를 얻을 수 있었다.

영문 초록

Anterior open bite is often caused by a downward rotation of the mandible and an excessive eruption of the posterior teeth. Skeletal anterior open bite had been considered to be one of the most difficult cases to treat in orthodontics. Especially, skeletal Class II anterior open-bite can be treated by either extrusion of incisors or intrusion of molars. Extrusion of anterior teeth is often undesirable for skeletal open-bite cases with a tendency of long-face. Skeletal Class II malocclusion with anterior open bite can be treated with skeletal anchorage system and orthognathic surgery may be avoided. Especially, maxillary molar intrusion is very effective for patients with increased anterior facial height and retrusive mandible because the treatment mechanism aids in reducing lower facial height and counterclockwise mandibular rotation. Vertical change of jaw leads to sagittal change. Therefore, additional distalization of maxillary and mandibular arch has to be planned; counterclockwise mandibular autorotation could change both lip support and lip protrusion. According to the amount and direction of tooth movement, miniplate was chosen for the maxilla, and miniscrew was chosen for mandible. This case report shows treatment of skeletal Class II malocclusion with anterior open bite and hyperdivergent facial profile. It is important to prepare appropriate skeletal anchorage according to the direction and amount of the tooth movement and considerations for retention and relapse should be included to maintain successful treatment results.
