코로나19 대응 경험에 기반한 K-방역모델의 국제표준화

국문 초록

코로나19는 2019년 12월 31일, 중국 우한의 WHO 사무소에서 처음으로 보고된 신종 감염병으로, 2020년말까지도 백신과 치료제와 같은 약물학적 수단이 없어 많은 확진자와 사망자가 발생하였다. 신종 바이러스에 의한 팬데믹(세계적 대유행)에 효과적으로 대응하려면 백신과 치료제가 개발되기 전까지 비약물학적 방역수단의 활용이 필요하다. 하지만 기존의 WHO의 지침과 ISO의 표준은 검사를 통해 빠르게 환자군을 찾아내고, 조기에 격리 및 치료하기 위한 기준으로 충분하지 않다. WHO의 국제보건규정을 포함한 지침은 제도와 정책적 차원의 권고가 많은 반면 코로나19와 같은 신종 감염병에 대응하려면 집단 및 지역사회 수준에서 적용 가능한 비약물학적방역수단의 개발과 활용이 필요하다. 본 연구는 코로나19 대응 경험에 기반한 K-방역모델의 국제표준화 추진을 위해서 시행되었다. 먼저 2020년2월부터 4월까지 국제기구가 배포한 지침과 국내외 비약물학적 방역정책을 조사 및 분석하였고, 자료에 근거해도출된 최소단위의 국제표준화 후보 항목을 유사성에 따라 통합하고, 정성적 지표인 ‘팬데믹 표준화 항목 적정성평가지표’로 통합한 후보 항목을 평가하였다. 최초 표준화 후보 항목은 28개였고, 팬데믹 표준화항목 적절성 평가지표로 평가한 결과 14개로 확정되었다. 확정된 14개의 항목은 정부가 추가한 4개 항목과 함께 2회에 걸쳐 방역 전문가그룹인 정책협의회에 의해 검토되었으며, 그 필요성과 타당성을 객관적으로 인정받아 한국의 ‘K-방역모델의 국제표준화 로드맵’에 반영되었다. 표준제안 우선순위에 따라 총 7종의 표준이 ISO에 제안되었으며, 팬데믹 대응용 표준개발을 전담하기 위한 작업반이 신설되었다. 앞으로 팬데믹 발생 주기는 더 짧아지고 그 위험도는 더 커질 것으로 전망하고 있다. 본 연구에서 도출된 팬데믹 대응용 비약물학적 표준화 항목이 인류의 팬데믹 대응 역량 향상에 기여할 수 있을 것으로 기대한다.

영문 초록

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is a highly contagious virus that causes coronavirus disease (COVID-19), the first confirmed case of which was reported by the World Health Organization in Wuhan, China. An active use of non-pharmaceutical prevention and control measures is essential for an effective response to the pandemic. This study aimed to derive international standardization items based on investigation and comparative analysis of non-pharmaceutical infection prevention and control measures implemented worldwide, including South Korea, between February and April, 2020, and assess these items by qualitative indicators to present standardization items required for pandemic response. This study collected the scientific evidence-based data regarding policies, procedures, and protocols for COVID-19 testing, tracking, tracing, and treatment from offline and online sources, including PubMed, Google, and Naver, and examined and analyzed such data. Based on the analyzed data, minimum units of internationally standardizable items were derived. After grouping similar items, the groups were assessed by the expanded “selection criteria for the development of pandemic standards” based on the ISO directive. Screening stations, social distancing, and temporary medical facility were initially included in the 28 new work items. These items were condensed to 14 items were subsequently confirmed from assessment based on the seven selection criteria. The 14 confirmed items were reviewed twice by a consultation group that included three infectious disease specialists, and in recognition of their necessity and validity, the items were reflected in the South Korean international standardization roadmap for pandemic response. With the public health system being paralyzed by unprecedented number of confirmed cases and deaths, an importance for international standard protocol that can be shared and compatible in all countries is growing. This study is the first to propose international standardization items for systematic pandemic response. The developed standardization items could be used not only in advanced countries but also in low- and middle-income countries to enhance the pandemic response capability of humans.
