고령 청각장애인의 일상생활활동 연구

국문 초록

목적 : 본 연구는 고령 청각장애인의 특성에 따른 일상생활활동의 차이 및 상관관계를 탐색하고자 하였다. 연구방법 : 본 연구는 2017년 장애인 실태조사 자료를 이용하였다. 장애인 실태조사에 참여한 총 6,549명 중 청각장애가 있는 노인 668명이 분석에 포함되었다. 일반적 특성은 나이, 성별, 가구소득, 최종학력, 장애등급, 보청기사용여부, 수화사용여부, 장애발생시기였고 일상생활 활동은 기본적 일상생활활동 12개 영역과 수단적 일상생활활동 8개 영역이 평가되었다. 분석은 두 집단 간 분석 또는 상관분석을 실시하였다. 연구결과 : 고령 청각장애인 여성은 남성보다 기본적 일상생활활동의 옷 갈아입기, 목욕하기, 보행, 이동, 배뇨, 그리고 수단적 일상생활활동의 물건사기, 약 챙겨먹기, 금전관리, 대중교통수단 이용하기에서 도움을 더 필요로 하였다(p<.05). 보청기를 사용하고 있는 고령 청각장애인은 보청기를 사용하고 있지 않는 고령 청각장애인에 비해 기본적 일상생활활동의 옷 갈아입기, 그리고 수단적 일상생활활동의 전화사용하기, 물건사기, 약 챙겨먹기, 금전관리, 대중교통수단 이용하기에서 도움을 더 필요로 하였다(p<.05). 일상생활활동 영역과 나이, 장애등급 사이의 상관분석 결과, 기본적 일상생활활동보다 수단적 일상생활활동의 상관계수가 더 높은 경향을 보였다. 결론 : 고령 청각장애인의 성별과 보청기사용여부에 따른 특정한 일상생활활동 영역의 차이가 나타나고 노화가 진행될수록 수단적 일상생활활동 영역의 문제가 더 심각함을 알 수 있었다. 이러한 결과를 근거로 고령 청각장애인의 일상생활 수행능력의 개선을 위한 지역사회 작업치료 중재 프로그램 개발이 필요함을 제안한다.

영문 초록

Objective : The aim of this study was to examine the comparison betweeen activities of daily living(ADL) according to characteristics of elderly people with hearing impairment, and the relationship between characteristics related to variables and ADL. Methods : This study used raw data of 2017 National Survey of the Disabled Persons. Among the total of 6,549 subjects, 668 elderly people with hearing impairment were included in the analysis. General characteristics were set as age, gender, family income, educational level, and disability grade, hearing aid, sign language, time point of presence of disability. The ADL was evaluated basic activities of daily living(B-ADL) 12 areas and instrumental activities of daily living(I-ADL) 10 areas. Statistical analysis was performed the and independent t test, chi-squre test, Mann-whitney u test, correlation analysis. Results : The older female with hearing impairment needed more help in dressing, bathing, gait, transfer, bladder control and I-ADL s shopping, drug management, money management and using transportation compared to male(p<.05). Hearing impairment person with hearing aid use needed more help B-ADL s dressing, and I-ADL s telephone use, shopping, drug management, money management and using transportation compared to hearing impairment person with hearing aid non-use(p<.05). As a result of correlation analysis between ADL area, age, and disability grade, the level of correlation coefficients of I-ADL areas was higher than that of B-ADL areas. Conclusion : According to gender and hearing aid of older adults with hearing impairment, differences of specific areas of ADL were found, and it was found that the problem of I-ADL were serious as the aging progressed than B-ADL. Based on these results, we suggest that community-based occupational therapy programs for the improvement of ADL performance in older adults with hearing impairment are needed.
