지역사회 데이터베이스를 활용한 한국 노인의 인지기능장애 진단현황과 장기요양보험 전문인력에 대한 연구

국문 초록

목적 : 본 연구는 지역사회 건강조사 데이터베이스를 활용하여 2011년부터 2016년까지의 생애전환기 노인 인구의 인지기능 진단 변화와 장기요양서비스 전문 인력 배치 변화의 추이를 정량적으로 제시하고자 실시하였다. 연구방법 : 연구 분석을 위해 질병관리본부의 2011~2016년까지의 지역사회 건강결과 및 건강 결정요인 데이터 베이스를 활용하였다. 분석대상은 생애전환기에 해당하는 전국 66세 노인인구의 인지기능장애에 대한 수검률, 진단율, 연도별⋅지역별 진단수 변화 등 노인 인지기능장애 진단에 관한 내용과 노인요양시설 전문인력 중 의사, 간호사, 작업⋅물리치료사, 요양보호사, 사회복지사의 인력배치 변화를 포함하였다. 연구결과 : 생애전환기 66세의 노인 인구의 인지기능장애와 관련하여 평균 77.7% 수검률을 보였고, 진단수의 변화는 지속적으로 상향 경향을 보였고, 평균 42.54%의 진단율을 보였다. 또한, 모든 시 ⋅도 단위를 포함한 진단 수의 전국 평균은 4538.50±537.85명으로 나타났다. 장기요양시설의 전문인력 배치의 연도별⋅직무별 분석 결과 평균 요양보호사 269219.67명, 간호사 10143명, 사회복지사 9332.50명, 작업⋅물리치료사 1740.17명, 의사 1271.17명 순으로 나타났다. 결론 : 연구 결과 노인의 건강관련의 검진율 및 노인의 인지기능 장애 진단율이 매해 큰 폭으로 늘어나고 있는 추세이며, 이와 달리 직접 서비스를 제공하는 재활인력 변화는 매우 점진적으로 변화하고 있음을 확인하였다. 본 연구를 토대로 향후 장기요양보험의 효과적인 재활서비스 전달체계의 제도적 방안 마련을 위한 기초자료로 활용될 것을 기대한다.

영문 초록

Objective : The purpose of this study was to quantitatively suggest that changes in the cognitive function diagnosis and the status of long-term care service experts in the transitional elderly population from 2011 to 2016, using community health survey. Methods : For the analysis, a database of community health status and health determinants from 2011 to 2016 was used. The subjects of the analysis - First, the diagnosis of cognitive dysfunction of the elderly, such as the rate of examination, diagnosis rate, and the number of diagnoses by year and region of the 66-year-old elderly population in the life-span transition period. Second, the long-term care facilities experts included changes in the workforce assignments of doctors, nurses, occupational⋅physical therapists, care helpers, and social workers. Results : In the 66-year-old elderly population, the average of inspection rate was 77.7%. The numerical change of diagnosis showed a continuous upward trend, and average diagnosis rate of 42.54%. The national average was 4538.50±537.85. According to the analysis of the yearly and professions of long-term care facilities, average care helpers were 269219.67, 10143 nurses, social workers were 9332.50, 1740.17 occupational and physical therapists, and 1271.17 doctors. Conclusion : As a result, the health screening rate of the elderly and the diagnosis rate of cognitive impairment of the elderly are increasing every year. However, it was confirmed that the change of rehabilitation experts who provide direct services is changing gradually. Based on this study, it is expected to be used as basic data for the preparation of an institutional plan for effective rehabilitation service delivery system of long-term care insurance.
