국문 초록

연구목적 : 본 연구의 목적은 지역사회를 중심으로 한 정신응급 초기 대응 내역을 대상으로 주요 요소별 분석 및 연도별 추이 분석을 통해 정신응급 대응체계의 개선을 위한 기초자료를 제공하는 데 있다. 방 법 : 2018년 1월 1일부터 2020년 12월 31일까지의 서울시 내 일개 지역 정신건강복지센터에서 정신응급출동 요청을 받은 총 180건을 대상으로 출동 발생 건수, 개입 시간, 의뢰 경로, 문제 종류, 대상자 요인, 조치사항에 대하여 기초통계분석 및 연도별 추이 분석을 실시하였다. 결 과 : 정신응급출동건수는 3개년 중 2019년에 가장 많았고, 건당 개입 소요시간은 2020년에 가장 높았다. 총건수로 볼 때에 의뢰 경로는 경찰이 가장 많았고, 자살과 정신증이 주요한 개입 문제였다. 조치사항으로는 보호자 인계 및 응급 입원이 많았고, 연도가 증가할 수록 복잡한 조치를 포함하는 경우가 늘어나고 있음이 관찰되었다. 그중에서도 2020년 응급 입원 경우 서울시 외 지역에 위치한 민간 병원으로 연계된 비율이 증가함을 확인할 수 있었다. 결 론 : 이 연구를 통하여 지역사회정신응급 대응을 주관하는 정신건강복지센터 역할의 중요성과, 효과적인 정신응급 위기 중재를 위해 서는 의뢰부터 조치까지 경찰 및 의료기관을 포함한 전체 구성원의 긴밀한 협조적 체계가 필요함을 확인할 수 있었다. 또한 COVID-19 팬데믹 상황이 정신응급조치에 영향을 미치는 것으로 보이는 바, 환경 변화에 유연한 대응을 위해 정신응급대응 시스템의 평가 및 보완 관련한 논의가 지속적으로 이루어져야 할 것이다.

영문 초록

Objectives : This study aimed to provide a database based on the characteristic analyses and yearly trend-analysis of the management of psychiatric emergency calls within the community to improve the psychiatric emergency management system. Methods : The study analyzed psychiatric emergency calls from mental healthcare centers in the districts of Seoul from January 1st, 2018, to December 31st, 2020. Based on the total 180 emergency requests, we conducted a statistical analysis and yearly trend statistics on the total number of dispatches, total invested time, request routes, problem types, and socio-de-mographic characteristics of patients and their management. Results : The total number of psychiatric emergency dispatches was highest in 2019 among the three years, and the invested time per dispatch was the longest in 2020. Based on the total number, request routes by police were the highest, and suicide and psychosis were the main problems. Guardian transfer and emergency hospitalization were most common, and we observed that more complicated interventions were necessary as the year went by. Especially in the case of emergency hospitalization in 2020, the admission rates in private hospitals outside of Seoul increased. Conclusion : Based on the study result, mental healthcare centers managing psychiatric emergency requests play crucial roles. All personnel from the police force to healthcare institutions should participate in kin cooperation to manage psychiatric emergency requests most effectively. Due to the impact of COVID-19, continuous evaluation and discussions on possi-ble improvements in the psychiatric emergency management system are necessary to provide more flexible and effective responses.
