국문 초록

목적: 서로 다른 네 가지 기기를 이용하여 측정한 수평각막윤부직경을 비교하고자 한다. 대상과 방법: 52명 52안을 대상으로 IOL Master 500, ANTERION, OA-2000, Pentacam HR을 이용하여 수평각막윤부직경을 측정하였다. 측정값은 반복측정 분산분석을 이용하여 비교하였고, 피어슨 상관관계 분석을 통해 상관관계를 확인하였다. Bland-Altman plot 을 이용하여 측정 기기 간 측정값 사이의 일치도를 확인하였다. 결과: IOL Master 500, ANTERION, OA-2000, Pentacam HR으로 측정한 수평각막윤부직경 측정값의 평균은 각각 11.65 ± 0.39, 11.52 ± 0.44, 11.51 ± 0.52, 11.28 ± 0.41 mm로 IOL Master 500이 가장 길었고, Pentacam HR이 가장 짧았다. IOL Master 500과ANTERION 사이에는 유의한 차이가 있었고(p=0.03), Pentacam HR은 다른 세 기기 모두와 유의한 차이가 있었다(각각 p<0.001). 기기 간 측정값 사이에는 양의 상관관계가 있었으며(모두 r>0.6, p<0.001), 95% 일치한계범위는 0.808에서 1.619 mm였다. 결론: IOL Master 500으로 측정한 수평각막윤부직경은 ANTERION, Pentacam HR과 유의한 차이가 있었고, OA-2000과는 유의한차이가 없었다. IOL Master 500과 나머지 세 기기 사이의 일치도가 낮아 임상적으로 대치하여 사용하기 어려울 것으로 생각된다.

영문 초록

Purpose: To compare the horizontal white-to-white (WTW) diameter measured using four devices. Methods: We measured the horizontal WTW diameter of 52 eyes of 52 patients using the IOL Master 500, ANTERION, OA-2000, and Pentacam HR devices. Repeated-measures analysis of variance was used to compare the measurements. Correlations between the measurements were evaluated using the Pearson correlation test. Agreement between measurements obtained using different instruments was confirmed using a Bland-Altman plot. Results: The mean horizontal WTW diameters obtained using IOL Master 500, ANTERION, OA-2000, and Pentacam HR were 11.65 ± 0.39, 11.52 ± 0.44, 11.51 ± 0.52, and 11.28 ± 0.41 mm, respectively. The horizontal WTW diameters obtained using IOL Master 500 and Pentacam HR were the largest and smallest, respectively. There was a statistically significant difference in mean diameter between IOL Master 500 and ANTERION (p = 0.03), as well as between Pentacam HR and the remaining three devices (all p < 0.001). There were positive correlations between the measurements obtained using the different devices (all r > 0.6, p < 0.001). The 95% limits of agreement between the measurements were 0.808-1.619 mm. Conclusions: There were statistically significant differences in horizontal WTW diameter between IOL Master 500 and ANTERION, and between IOL Master 500 and Pentacam HR, but not between IOL Master 500 and OA-2000. Measurements obtained using IOL Master 500 and the remaining three devices are not interchangeable because of their low level of agreement.
