초로기 치매환자 비약물프로그램 개발을 위한 작업치료사의 요구도 조사

국문 초록

목적: 2021년도 대한작업치료사협회 보건복지부 치매전문교육에 참여한 작업치료사를 대상으로 초로기 치매환자에 대한 인식과 프로그램 요구도 조사를 분석하여 향후 초로기 치매환자를 위한 프로그램 개발에 활용하기 위한 기초자료를 제시하고자 한다. 방법: 초로기 치매환자 특성 및 비약물 프로그램에 대한자료를 수집한 후 분석하였다. 자료내용을 기반으로 설문지를 작성하고, 결과를 분석하여 초로기 치매환자를 위한프로그램 개발에 활용하기 위한 자료를 제시하였다. 결과: 요구도 분석 결과 초로기 치매환자와 관련된 교육경험이 없는 작업치료사가 많았으며, 초로기 치매환자를위한 서비스가 없는 기관이 많았다. 초로기 치매환자 프로그램을 운영하지 못하는 이유는 ‘대상자 모집의 어려움, 시설 부족, 진행인력 부족, 초로기 환자에 대한 이해부족’ 순이었다. 조사대상자 대부분이 초로기 치매환자 프로그램이 필요하다고 하였으며, 프로그램에서 중요하게 다루어져야할 영역으로 ‘일상생활, 인지, 보호자 교육, 사회활동, 정신행동증상 조절, 신체영역’ 순이었다. 결론: 초로기 치매는 빠르게 진행되기 때문에 초기부터증상과 일상생활 및 사회생활 수행능력 저하를 늦추기 위한적극적인 개입과 프로그램 개발이 필요하다. 초로기 치매환자에 대해 이해할 수 있는 교육에 대한 요구가 높았으나현재 초로기 치매환자와 관련된 교육 경험이 없는 작업치료사의 비율이 높다. 따라서 작업치료사의 초로기 치매에 대한 전문성을 높이기 위한 교육과정 개발이 필요하며, 이를위한 제도적인 뒷받침이 되어야 할 것이다.

영문 초록

This study aimed to analyze a survey on the perception and necessity of programs for patients with elderly onset dementia (EOD), among occupational therapists who participated in 2021 dementialized education by KAOT. Data on the characteristics and non-pharmacological programs for patients with EOD were collected. A questionnaire was created based on the data, and the survey was conducted among occupational therapists and analysis undertaken. As a result of the analysis, many occupational therapists were inexperienced in EOD patients' education, and most of the institutions were not available for service. The reasons why EOD patients' programs were inoperable are difficulties in subject recruitment, followed by lack of facilities, staff, and understanding of EOD patients' needs. Most of the surveyed subjects stated the need for the program focussing on EOD patients and mentioned activities of daily life (ADL), followed by cognition, caregiver education, social activity, modulation of psychomotor symptoms, and physical activities as important areas. Since EOD progresses rapidly, active intervention with specifically developed program in the early stages are necessary to delay symptoms and prevent deterioration in ADL and social performance skills. Despite the high demand for education about EOD patients, a high percentage of occupational therapists are inexperienced about it. Therefore, curriculum development is necessary to increase the professionalism of occupational therapists in EOD, and institutional support should be provided.
