백내장수술 후 발생한 급성 유리체황반견인증후군의 자연 소실

국문 초록

목적: 백내장수술 후 발생한 유리체황반견인의 자연 소실 1예를 보고하고자 한다. 증례요약: 70세 여자 환자가 우안 백내장에 대해 우안 수정체초음파유화술 및 인공수정체삽입술을 시행 받았다. 수술 전에 시행한 빛간섭단층촬영(optical coherence tomography)검사에서 우안 유리체가 황반부에 유착된 소견이 보였고, 소수점 시력(decimal visual acuity) 기준으로 당시 우안 나안시력은 0.63, 최대교정시력은 0.8로 측정되었다. 백내장수술 후 1일째 빛간섭단층촬영검사에서 빛수용체 타원체구역(ellipsoid zone) 및 외경계막(external limiting membrane)의 연속성이 소실된 우안 유리체황반견인이 관찰되었고, 우안의 최대교정시력은 0.32로 수술 전보다 낮게 측정되었다. 수술 후 2주일째 빛간섭단층촬영에서 황반부 유리체황반견인이 자연소실되었지만 유리체황반유착은 지속되는 것이 관찰되었다. 수술 후 4개월째 경과 관찰에서 변시증 없이 최대교정시력이 0.8로 상승되었다. 결론: 백내장수술 후에 급성 유리체황반견인이 발생할 수 있으며, 유리체황반유착의 분리 없이 유리체황반견인이 자연 소실될 가능성이 있으므로 발생 초기에는 수술 치료보다는 면밀한 경과 관찰을 고려해볼 수 있다.

영문 초록

Purpose: To report a case of spontaneous resolution of vitreomacular traction (VMT) syndrome after cataract surgery. Case summary: A 70-year-old woman underwent lens emulsification and intraocular lens implantation for a right-eye cataract. Preoperative optical coherence tomography (OCT) showed vitreomacular adhesion (VMA). At that time, the decimal visual acuity of the right eye was 0.63 and the best-corrected visual acuity (BCVA) was 0.8. At one day after cataract surgery, OCT showed VMT, with disruption of the ellipsoid zone and outer limiting membrane. The BCVA of the right eye was 0.32, which was worse than before surgery. At two weeks after cataract surgery, OCT revealed spontaneous resolution of the VMT, whereas the VMA persisted. At four months after cataract surgery, the BCVA of the right eye was 0.8. Conclusions: In cases of VMT after cataract surgery, there is a possibility of spontaneous resolution without VMA separation. Therefore, careful observation in the early stages should be considered before surgical treatment.
