장기 혈액투석이 당뇨망막병증환자의 시력과 중심망막두께에 미치는 영향

국문 초록

목적: 장기 혈액투석이 당뇨망막병증환자의 시력과 중심망막두께에 미치는 변화를 살펴보았다. 대상과 방법: 2008년 1월 1일부터 2018년 12월 31일까지 당뇨망막병증을 진단받고 만성콩팥병증으로 혈액투석을 받은 환자의 의무기록을 분석하였다. 대상 환자는 혈액투석을 주 3회, 18개월 이상 받은 환자들 중 혈액투석을 시작하기 6개월 이전에 당뇨망막병증을 진단 받을 자로, 시력에 영향을 미치는 유리체출혈 발생, 다른 망막질환 동반, 혈액투석 후 안내수술을 받은 환자는 제외하였다. 혈액투석 전, 혈액투석 후 1, 3, 6, 12, 18개월째 시력, 중심망막두께를 분석하였다. 결과: 본원에서 당뇨망막병증을 진단 받고 당뇨로 인한 만성콩팥병증으로 혈액투석을 받은 111명 222안 중, 혈액투석 후 당뇨망막병증이 진단된 174안을 제외하였다. 혈액투석 시작 전 3개월 이내 유리체출혈이 발생한 10안, 망막전막이 동반된 2안, 혈액투석 후 백내장수술을 받은 4안을 제외하고 18명 32안이 포함되었다. 평균 나이는 53.71 ± 9.25세, 남성 24안, 여성 8안이었다. LogMAR 시력은 혈액투석 전, 혈액투석 18개월 후에 각각 0.36 ± 0.28, 0.28 ± 0.27로 유의하게 향상되었다(p=0.002). 평균 중심망막두께는 혈액투석 전, 혈액투석 12개월 후에 각각 307.12 ± 89.52 μm, 279.71 ± 61.75 μm로 유의하게 감소하였다(p=0.02). 결론: 장기적인 혈액투석을 받은 당뇨망막병증환자에서 투석 전에 비하여 중심망막두께가 감소하고 시력이 호전되었다.

영문 초록

Purpose: To investigate the changes of visual acuity and central macular thickness (CMT) in patients with diabetic retinopathy(DR) receiving long-term hemodialysis (HD). Methods: From January 1, 2008, to December 31, 2018, the medical records of patients who were diagnosed with DR receiving HD three times a week for ≥18 months due to chronic kidney disease (CKD) were analyzed. Among them, patients diagnosed with DR 6 months before the start of HD were included. Patients with vitreous hemorrhage (VH) affecting visual acuity (VA), other retinal diseases, and cataract surgery after HD were excluded. The VA and CMT before HD and at 1, 3, 6, 12, and 18 months after HD were analyzed. Results: Of the 222 eyes of 111 patients who were diagnosed with DR and received HD for CKD due to diabetes, 174 eyes with DR diagnosed after starting HD were excluded. Ten eyes with VH before starting HD, two eyes with epiretinal membrane, and four eyes with cataract surgery after starting HD were also excluded. Thirty-two eyes of 18 patients were included. The mean age of the patients was 53.71 ± 9.25 years. Twenty-four males and eight female patients were included in the study. The mean logMAR VA improved significantly from 0.36 ± 0.28 before starting HD to 0.26 ± 0.27 at 18 months after starting HD (p = 0.002). The mean CMT was significantly decreased from 307.12 ± 89.52 μm before starting HD to 279.71 ± 61.75 μm at 12 months after starting HD (p = 0.02). Conclusions: In patients with DR who underwent long-term HD, CMT decreased and VA improved, when compared with these parameters before HD.
