국문 초록

목 적：Knee PA Flexion 검사를 위한 보조 기구를 제작하여, 관절염 추적검사의 일관성을 높이고 정확한 Knee joint Space 측정을 위한 유용성을 평가하고자 한다. 방 법： 2017년 1월부터 11월까지 무릎 관절염으로 내원한 환자 중 인공 슬관절 전치환술(Total Knee Replacement)을 시행하지 않은 60명(여30명, 남30명, 평균연령 51세)을 대상으로 방사선 투과성 소재로 된 자체 개발 보조 기구 적용 전, 후로 나누어 일반 영상 검사를 실시하였다. Medial Femorotibial Joint를 PACS를 이용하여 측정하였고 각 자료를 비교하여 분석 평가하였다. 결 과：보조 기구 적용 전, 후 Medial Femorotibial Joint간격 측정 데이터를 통계 분석한 결과, 적용 전 20.88±7.46 mm에서 적용 후 37.45±7.45 mm로 평균값이 16.57 mm 증가되었으며 보조기구 사용에 따른 유의성이 나타났다(p<0.001). 결 론：본 연구에서 고안된 보조 기구는 보다 정확한 Tibia Plateau를 묘출함으로써 Medial Femorotibial Joint를 일관성 있고, 정확하게 측정할 수 있었다. 또한 자세 유지가 힘든 Knee PA Flexion 검사에서 보조 기구를 적용하여 환자 안전을 확보함으로써 검사보조자의 불필요한 방사선 피폭을 줄일 수 있다.

영문 초록

Purpose：The purpose of this study is to evaluate the utility of the self-developed ancillary device to improve the consistency of follow-up evaluation for arthritis during Knee PA Flexion test, and to in-crease the Knee joint Space measurement by over-lapping the Tibia Plateau. Targets and Methods：From January to November 2017, the study consisted of 60 patients, composed of 30 females and 30 males, in the mean age of 31 years, who did not undergo total knee replacement surgery, and who were referred to follow-up evaluation 2 or more times for the diagnosis of knee arthritis. Subjects underwent simple radiography before and after applying the ancillary device which is made of acryl and PE foam. PACS (Marotech marosis view version 5.4) was used to measure the Medial Knee Joint Space (mm). The obtained data from before and after applying the ancillary device were analyzed by Pair Sample T-test. Result：As a result, the Medial Knee Joint Space after applying the ancillary device (37.45±7.45mm) was 16.57 mm higher, in average, than the measurement before applying the ancillary device (20.88±7.46 mm), showing that there is a significant relationship between the use of the ancillary device and the Medial Knee Joint Space measurement Conclusion：The study confirmed that the specially designed ancillary device in this study measures wider Medial Knee Joint Space, by overlapping the Tibia Plateau, and it can increase the diagnostic value via improving consistency of follow-up evaluation. Also, it increases the stability while reducing unnecessary radiation exposure and re-inspection in patients who has difficulty maintaining the posture due to advanced arthritis.
