알파차단제를 복용하는 환자의 백내장수술 시 홍채이완증후군에 대한 호마트로핀 점안제의 효과

국문 초록

목적: 알파차단제를 복용하는 환자에서 백내장수술 시 발생할 수 있는 홍채이완증후군을 예방하기 위해 수술 전에 사용하는 호마트로핀 점안제의 효과에 대해 분석하였다. 대상과 방법: 알파차단제를 복용하고 백내장수술을 받은 68명 98안을 대상으로, 수술 전 호마트로핀을 점안한 1군, 아트로핀을 점안한 2군과 대조군(3군)으로 나누어 연령, 성별, 기저질환, 알파차단제의 종류, 수술 전, 중 및 후 동공 크기, 수정체유화술의 역동학변수 및 수술합병증을 비교하였으며, 또한 수술 중 홍채이완증후군의 발생 및 정도와 관련된 위험 인자를 분석하였다. 결과: 세 군 간 연령, 기저질환의 유무에 유의한 차이는 없었다. 세 군 모두 tamsulosin이 알파차단제 종류 중 가장 많았다. 수술 전후 동공 크기와 수정체유화술의 소비 에너지는 1군과 2군이 대조군에 비해서 유의한 차이를 보였다. 수술 1일째엔 1군과 2군 사이에도 유의한 차이는 보였다. 세 군 간 각막절개로의 홍채 탈출 소견의 발생엔 유의한 차이를 보였으나, 그 외 소견이나 합병증 발생엔 유의 한 차이는 없었다. 홍채이완증후군의 정도 또한 1군과 2군이 대조군보다 낮았는데, 수술 전 동공 크기와 아트로핀 점안이 그 정도를 낮추는 인자였다. 결론: 홍채이완증후군 정도를 낮추기 위해 예방적으로 시행한 호마트로핀 점안법은 기존에 알려진 아트로핀 점안법과 비슷하게 그 정도를 경감시켰고, 수술 후 동공 크기의 회복은 더 빨랐다. 수술 전 호마트로핀 점안이 알파차단제를 복용하는 환자의 홍채이완증후군 정도를 낮출 수 있는 선택 사항이 될 수 있을 것으로 사료된다.

영문 초록

Purpose: To investigate the effects of homatropine eye drops before cataract surgery to prevent intraoperative floppy iris syndrome (IFIS) in patients treated with alpha blockers. Methods: The medical records of 98 eyes from 68 patients taking alpha blockers who underwent cataract surgery were retrospectively reviewed and divided into three groups: patients with preoperative instillation of homatropine (group 1), patients with preoperative instillation of atropine (group 2), and patients without preoperative eye drops (group 3) for three days. Age, sex, underlying disease, type of alpha blocker, preoperative and postoperative pupil size, parameters of phacoemulsification, surgical complication, and the degree of IFIS of patients were measured to compare among the three groups. Results: No significant differences were found in age and underlying diseases. Significant differences were found in the pre-operative and postoperative pupil size, cumulative dissipated energy, and the incidence rate of iris prolapse through the incision sites among the three groups, but no significant difference was detected in other findings, or with regard to intraoperative or post-operative complications. In addition, the preoperative pupil size and preoperative instillation of 1% atropine were significant factors for decreasing the degree of IFIS. Conclusions: Preoperative instillation of homatropine as well as atropine before cataract surgery minimized the degree of IFIS in patients taking alpha blockers. In addition, the pupil size recovered more rapidly after surgery in the homatropine group than in the atropine group.
