무릎 과간와(Intercondylar fossa) 검사시 변형된 Supine 자세에서 무릎 각도 변화에 따른 유용성 평가

국문 초록

목 적 : 전방십자인대 파열을 주소로 내원한 환자에게 시행되어지는 방사선학적 검사 중 기본적으로 검사하는 Knee tunnel view에서 자세의 어려움을 보완하고자 prone position으로 이루어지는 기존 Camp Coventry 검사법과 supine sosition에서 무릎 각도 변화에 따른 변형된 과간와 검사의 영상을 비교 분석하여 그 유용성을 알아보고자 하였다. 대상 및 방법 : 2014년 9월부터 11월까지 무릎에 병변이 없는 정상 지원자 성인 남녀 23명을 대상으로 하였고, 연령군은 남자 15명, 여성 8명 이었으며 총 평균나이는 30세 이었다. 영상 분석은 기존 검사방법인 Camp Coventry method 영상 1장과 supine position에서 무릎 각도 변화에 따른 120°, 130°, 140°, 150°의 Modified method 영상 4장을 정형외과 전문의 2명, 방사선사 3명이 4개의 설문 문항으로 리커트 5점 척도 (매우좋음 5점, 좋음 4점, 보통 3점, 나쁨 2점, 매우나쁨 1점)로 평가하였다. 평가자간 신뢰도를 위해 스피어만 상 관분석하였으며, Camp Coventry 검사와 Modified method가 동일한지 알아보기 위해서 Wilcoxon 부호순위검정을 실시하여 동질성 검정을 실시하였다(P>.05). 결 과 : 설문문항 (A)“Intercondylar fossa가 명확하게 보이는가?” 에서는 CC법과 130° 검사법이 통계적으로 동질한 것으로 나타났다(P>.05). 평균값은 Camp Coventry법이 3.65, 130°법이 3.87으로 130°법이 0.22가 더 높게 나타났다. 설문문항 (D)“A),B),C) 설문평가 지표를 참고 하였을 때 영상 중 가장 진단적 가치 있는 영상은?” 종합적인 평가질문에서 CC법과 130°법이 통계적으로 동질한 것으로 나타났다(P>.05). 평균값은 Camp Coventry법이 4.22, 130°법이 4.57로 130°법이 0.35가 더 높게 나타났다. 결 론 : Knee Tunnel View 검사에서 기존의 Camp Coventry 검사법 보다 supine position에서 무릎 각도 130° Modified method가 십자인대파열 환자의 재검사를 줄여 방사선 피폭을 경감할 수 있었고 재건수술 후 추적검사 시 환자가 최소한의 움직임으로 편안하게 검사를 할 수 있으므로 보다 더 정확한 검사 결과를 제공할 수 있을 것으로 사료된다.

영문 초록

Purpose : Among radiological examinations to patients with anterior cruciate ligament rupture, Knee tunnel view, the basic examination, has difficulty in posture so that it objects to figure out the usefulness by comparing images of Camp Coventry examination consisting of prone position and modified intercondylar fossa examination by change of knee degree in supine position. Materials and Methods : Normal 23 adult men and women without any disease in knee were intended from September to November 2014 and age group was 15 men and 8 women, and the average age was 30. Image analysis evaluated 1 image of Camp Coventry, the established examination method, and 4 images of modified method in supine position by degree of knee 120°, 130°, 140°, 150° by 2 orthopedists and 3 radiogical technologists with 4 questions by lickertis 5 scale(Very good 5 points, Good 4 points, Normal 3 points, Bad 2 points and Very Bad 1 points). Spearman rank-order Correlation coefficient is analyzed for credibility between evaluator and to inquire the identity between Camp Coventry examination and Modified method, they went through homogeneity examination by Wilcoxon marched pairs signedranks test(P.05). Result : In question “Is intercondylar fossa seen apparently?”, Camp Coventry method and 130° modified method were homogeneous statistically(P>.05). The average value was 0.22 higher in Camp Coventry method by 3.65, and 130° modified method by 3.87. In question “Referred to indicators of question, what is the most diagnostically valuable image?”, the comprehensive evaluation question, Camp Coventry method and 130° modified method were homogeneous statistically(P>.05). The average value was 0.35 higher in 130° modified method by 4.57 and Camp Coventry method by 4.22. Conclusion : In knee tunnel view examination, 130° modified method of supine position can alleviate radiation exposure of ACL patient decreasing reexamination and it can be done comfortably with minimum movement of patient after reconstruction surgery. It able to provide more precise examination result.
