폐쇄성수면무호흡증후군에서 Videofluoroscopy의 임상적 유용성

국문 초록

목 적 : 폐쇄성수면무호흡증후군의 외과적 치료를 위해 폐쇄부위를 정확히 확인하는 것은 수술방법을 결정하는데 매우 중요하다. Videofluoroscopy를 근거로 파악한 폐쇄부위와 투시소견을 바탕으로 수면다원검사 결과를 비교분석하여, 상기도 폐쇄부위의 해부학적 특성 평가 및 수술적 지표로써 임상적 유용성을 알아보고자 하였다. 대상 및 방법 : 코골이 및 수면중무호흡을 주 증상으로 내원하여 폐쇄성수면무호흡증 의심 하에 수면다원검사와 Videofluoroscopy 검사를 시행한 18세 이상의 성인 30명을 대상으로 하였다. 수면다원검사의 Apnea-Hypopnea Index를 기준으로 각 지표와 Videofluoroscopy 의 상기도 폐쇄부위와 투시소견을 바탕으로 변인을 분석하여 비교 평가하였다. 결 과 : 성별에 따른 분석에서 남자가 여자에 비해 4배 많았다. BMI, Mean SpO 2 , Minimum SpO 2 , AHI에서는 유의한 차이가 없었다( p >0.05). AHI별 특성분석에서는 Age, Height를 제외한 항목에서 유의한 차이를 보였다( p< 0.05). AHI 분류별 폐쇄부위에 따른 결과 분석에서는 단일폐쇄 및 복합폐쇄 유무에 상관없이 유의한 차이를 보이지 않았다 ( x 2 =11.222, p= 0.082). AHI 분류별 투시소견에 따른 결과에서는 AHI지수에 따른 투시소견은 유의한 차이가 있었다( x 2 =16.770, p= 0.010). 결 론 : Videofluoroscopy는 폐쇄성수면무호흡증후군의 원인이 될 수 있는 폐쇄부위와 전체 상기도의 해부학적 특성을 잘 관찰할 수 있으며, 나아가 수술적 지표를 평가할 수 있는 유용한 검사이다.

영문 초록

Purpose : The detection of obstruction site is very important for the surgical treatment of obstructive sleep apnea(OSA). Were compared analyzing the results of polysomnography based on closed portion and perspective findings grasp the Videofluoroscopy grounds. The above was also Let s examine the clinical utility as anatomical characterization and surgical indication of the closure site. Materials and Methods : Thirty who had been enrolled for the suspicion of OSA were studied. They were examined by videofluoroscopy and polysomnography to evaluate the obstruction site. The videofluoroscopy was analyzed by obstruction level and videofluoroscopy findings and its characteristies were evaluated on the basis of the results in polysomnography and videofluoroscopy. Result : In the analysis based on gender, men were four times greater than that of the female. BMI, Mean SpO 2 , Minimum SpO 2 , significant difference in AHI was no ( p >0.05). In AHI another characterization Age, showed a significant difference in items except Height and ( p >0.05). Was the analysis of the results according to AHI categorical closure site, with or without the composite closure with a single closure, there was no significant difference ( x 2 =11.222, p =0.082). The results based on AHI categorical perspective findings perspective findings based on AHI index was a significant difference ( x 2 =16.770, p =0.010). Conclusion : The analysis of videofluoroscopy may be a useful tool for detecting the obstruction site in OSA.
