국문 초록

본 연구는 치매환자의 가족을 대상으로 치매환자 재가돌봄의 구조모형을 실증적으로 분석함으로써 체계적 치매관리와 가족차원의 부담경감, 지역사회중심의 재가돌봄을 위한 정책적․실천적 함의를 도출하기 위해 선행연구와 실증적 모형으로서의 앤더슨행동모형(ABM)과 이론적 구성 틀로서의 계획 행동이론(TPB)을 통합하여 연구모형을 구축하였다. 재가돌봄이라는 행동의 의향과 결정의 구조를 밝히기 위해 ‘선행요인(독립변인) → 행동의향(매개변인) → 재가돌봄행위(종속변인)’의 이론적 틀에 치매특성수준, 가족기능수준, 돌봄 부담수준을 각각 소인요인, 가능성요인, 욕구요인을 대변하는 독립변인으로 설정하고, 태도, 규범, 지각행동이라는 하위요인으로 구성된 행동의향을 매개변인으로 통합하여 재가돌봄행위라는 종속변인으로 설정하여 하나의 경로모형으로 구축하였다. 이러한 경로모형 하에 사회적 지지를 조절변인으로 설정하여 구조적 경로(혹은 인과적 효과)의 차이를 검증하였다. 본 연구의 결과를 통해 돌봄부담수준이 재가돌봄에 영향을 미치는 가장 중요한 변수로 중요성이 입증되었으며, 돌봄부담의 행동의향에 대한 부정적 영향을 낮추고, 행동의향의 재가돌봄행위에 대한 효과를 높이기 위한 사회적 지지의 중요성이 확인되었는데 이는 국가차원의 정책적 접근 필요성에 대한 시사점을 준다.

영문 초록

The purpose of this study is to find political and practical implications for systematic dementia management, burden reduction among families and home care in communities by analyzing a structural model of dementia patient home care among families with dementia patients empirically. To achieve this purpose, advanced research reviewed (was carried out), (and) Anderson’s Behavioral Model(ABM) as an empirical model, and Theory of Planned Behavior(TPB) as a theoretical framework were combined to build a research model. In order to determine the structure of intention and decision for home care, within a theoretical framework of ‘antecedent(independent variable) → behavioral intention(mediating variable) → home care behavior (dependent variable)’, a level of dementia characteristics, a level of family functions and a level of care burden were appointed as independent variables representing a predisposing factor, a functionality factor and a needs factor respectively. Also, behavioral intention consisting of sub-factors; attitude, norm and perceived behavior was combined with mediating variables and it was appointed as a dependent variable, home care behavior to build a path model. Based on this path model, social support a moderating variable and the difference from a structural pathway(or causal effect) was verified. The findings of this study prove the importance of a level of care burden as the most significant variable affecting home care, and demonstrate that social support is important to reduce a negative effect of care burden on behavioral intention and to improve the effect of behavioral intention on home care behavior.
