홍채고정 안내렌즈삽입술 후 장기간 경과관찰에 따른 각막내피세포수의 감소 3예

국문 초록

목적: 근시환자에서 홍채고정 안내렌즈(ArtisanⓇ lens) 삽입 후 장기간 경과관찰에서 심한 내피세포수 감소가 있었던 3예를 보고하고자 한다. 증례요약: 32세 여성이 양안의 근시로 홍채고정 안내렌즈삽입술을 시행받았다. 술 전 각막내피 세포밀도는 우안 2,556 cells/mm2, 좌안 2,674 cells/mm2였으나, 4년 후 경과관찰에서 우안 1,968 cells/mm2, 좌안 1,997 cells/mm2로 감소하였다. 양안 홍채고정 안내렌즈삽입술을 시행받은 27세 여성은 술 전 각막내피 세포밀도가 우안 3,222 cells/mm2, 좌안 3,122 cells/mm2였으나, 4년 8개월 후 우안 2,729 cells/mm2, 좌안 2,488 cells/mm2로 감소하였다. 타 안과에서 양안 안내렌즈삽입술을 시행받은 후 좌안의 안압상승으로 당일 안내렌즈를 제거한 후 전원된 39세 여성의 좌안에 홍채고정 안내렌즈삽입술을 시행하였다. 술 전 각막내피 세포밀도는 좌안 2,500 cells/mm2였으나, 8년 후 방문 시 1,873 cells/mm2로 감소되어 있어, 홍채고정 안내렌즈를 제거하고 백내장수술을 시행하여 6개월째 내피세포수는 1,412 cells/mm2였다. 결론: 홍채고정 안내렌즈삽입술은 안전하고 효과적인 근시교정 방법이지만 4년 이상 장기간 경과관찰을 통한 각막내피세포밀도 검사가 필요할 것으로 생각된다.

영문 초록

Purpose: To report three cases of severe endothelial cell density loss after iris claw phakic intraocular lens (Artisan？？ lens) implantation. Case summary: A 32-year-old woman underwent iris claw intraocular lens implantation in both eyes. Preoperative corneal endothelial cell count was 2,556 cells/mm2 in the right eye and 2,674 cells/mm2 in the left eye. After 4 years, the corneal endothelial cell count was 1,968 cells/mm2 in the right eye and 1,997 cells/mm2 in the left eye. A 27-year-old woman underwent iris claw intraocular lens implantation in both eyes. Preoperative corneal endothelial cell count was 3,222 cells/mm2 in the right eye and 3,122 cells/mm2 in the left eye. After 4 years 8 months, the corneal endothelial cell count was 2,729 cells/mm2 in the right eye and 2,488 cells/mm2 in the left eye. A 39-year-old woman underwent phakic intraocular lens implantation in other clinic, and the lens of left eye was removed the same day because of elevated intraocular pressure. She underwent iris claw intraocular lens implantation in the left eye. Preoperative corneal endothelial cell count was 2,500 cells/mm2 in the left eye, which decreased to 1,873 cells/mm2 after 8 years. Six months after intraocular lens removal and cataract surgery, her cornea endothelial cell count was 1,412 cells/mm2. Conclusions: Although iris-fixed intraocular lens implantation is safe and effective for correcting myopia, at least 4 years of long-term observation for evaluating corneal endothelial cell density maybe necessary.
