하악 제3대구치 판개(Opercula)의 병리조직학적 연구

영문 초록

The author studied 33 cases of the opercular histopathologically obtained from a position overlying impacted or partial erupted 3rd molards during extraction of these teeth. These soft-tissue specimens (opercular) were obtained from 33 male & females patients aged from 18 to 30 year old who came to have wisdom teeth removal at the Department of Oral Surgery, Infirmary of Dental College, Yonsei University. All findings were based on the original six slides cut with a thickness of 6 micron and stained with hematoxylin and eosin. The histopathologic findings were tabulated and discussed on the epithelial, connective tissue, and vascular changes of opercula. The results of this studies are as follows: 1. The mucosal epithelial cell covering the operculum was found squamous in 31 cases (96.88%) and odontogenic type in 17 cases (53.13%) of 32 cases but for one with total ulceration of epithelium. Acanthosis was present in 24 cases (75.00%) and also accompanied by pseudoepitheliomatous hyperplasia in 6 cases (25.00%). 2. Hyperkeratosis or hyperparakeratosis in the superficial layers of the epithelium in 26 cases (81.25%) was found, but the dyskeratosis was shown 6 cases (18.75%) and 2 cases (33.33%) of them revealed benign epithelial pearls. 3. In 32 cases, opercular showed 17 cases (53.13) of spongiosis, 5 cases (15.63%) of vesiculation, and 1 case (3.13%) of atrophy in epithelium. Partial or total ulceration of epithelium was present in 12 cases (36.36%) of 33 opercula. 4. Squamous and odontogenic-type epithelium were able to observe simultaneously in 16 cases (50.00%), especially the odontogenic type showed metaplastic reaction to squamous type in the are of junction between the inner surface and the outer surface of operculum. 5. Epithlium of the odontogenic type was found deep within connective tissue layer and it composed of the epithelial islands in 1 case (3.03%). 6. Of 33 cases opercula, the osteoid or bone was only present in 3 cases (9.09%) and 2 cases (66.67%) showed the osteoblasts around and osteocytes in bony tissue. 7. In connective tissue, myxomatous change was found in 2 cases (6.06%) and eosinophils were increased in 7 cases (21.21%). 8. Salivary gland tissues were found in 2 cases (6.06%), they were all mucous type glands, and ectopic sebaceous gland tissue was also found in 1 case (3.03%). 9. Infiltration of inflammatory cell in connective tissue was found in 27 cases (81.81%) and fibrosis was shown in 17 cases (51.52%) of 33 cases. 10. Vascular changes in 33 cases revealed the following findings; vascular proliferation in 12 cases (36.36%), hyperplastic changes in 6 cases (18.18%), fibrosis of adventitia in 4 cases (12.12%) and sclerotic changes in 1 cases (3.03%). 11. Opercula contained a host of different tissues. Therefore it may conclude that many of these opercula have the propensity to undergo neoplastic changes, or cause postoperative complications.
