단안 망막정맥폐쇄 환자의 무증상 반대안의 사상판 두께

국문 초록

목적: 단안 망막정맥폐쇄 환자의 무증상 반대안과 정상안 대조군의 사상판 두께를 비교하였으며, 망막정맥폐쇄의 종류에 따라 사상판 두께를 비교하였다. 대상과 방법: 단안 망막정맥폐쇄 환자 40명과 정상안 환자 35명을 대상으로 연구를 시행하였으며, 망막정맥폐쇄 증상안과 무증상안, 단안 망막정맥폐쇄 환자의 무증상 반대안과 정상안, 증상안의 망막정맥폐쇄의 종류에 따른 무증상안의 사상판 두께를 각각 비교하였다. 사상판 두께는 스펙트럼영역 빛간섭단층촬영의 Enhanced depth imaging 방법을 이용하여 중심 사상판 두께를 측정하였다. 결과: 단안 망막정맥폐쇄 환자의 무증상 반대안과 증상안의 사상판 두께는 각각 204.13 μm, 211.33 μm로 통계적으로 유의한 차이는 없었다( p =0.202). 그러나 무증상 반대안의 사상판 두께는 정상안의 사상판 두께 217.76 μm에 비해 유의하게 감소되어 있었다 ( p =0.046). 망막정맥폐쇄의 종류에 따른 단안 망막정맥폐쇄 환자의 무증상 반대안의 사상판 두께는 통계적으로 유의한 차이가 없었다 ( p =0.672). 결론: 단안 망막정맥폐쇄 환자의 무증상 반대안은 정상안에 비해 사상판의 두께가 감소되었다. 이로 보아 녹내장뿐만 아니라 망막정 맥폐쇄에도 사상판 두께가 관여할 수도 있을 것으로 추측할 수 있다.

영문 초록

Purpose: To compare the lamina cribrosa thickness in fellow eyes of patients with unilateral retinal vein occlusion (RVO) with the normal control eyes and the type of RVO. Methods: This study included 40 patients with unilateral RVO and 45 normal control subjects. We compared the lamina cribrosa thickness between the RVO eyes and the fellow eyes, the fellow eyes and the normal control eyes and the type of RVO eyes. We measured central lamina thickness using enhanced depth imaging spectral-domain optical coherence tomography. Results: In patients with unilateral RVO, central lamina cribrosa thickness was not significantly different between the RVO eyes (211.33 μm) and the fellow eyes (204.13 μm; p = 0.202). However, central lamina cribrosa thickness in the fellow eyes was sig-nificantly reduced compared with the normal control eyes (217.76 μm; p = 0.046). Central lamina cribrosa thickness in the fellow eyes according to the type of RVO was not statistically significantly different (p = 0.672). Conclusions: This study showed that the central lamina cribrosa thickness in the fellow eyes of patients with unilateral RVO was thinner than in normal patients. Therefore, the lamina cribrosa thickness may be associated with RVO as well as glaucoma.
