병원간호사 서비스 교육에 대한 생명윤리적 고찰

국문 초록

오늘날 의료 환경의 변화는 의료기관으로 하여금 경쟁력 강화와 성과 달성을 목표로 고객만족을 위한 경영전략을 필수적으로 적용하게 만들었다. 이에 따라 고객만족을 위한 서비스 교육이 최근에는 거의 모든 의료기관에서 간호사를 포함하여 전 직원을 대상으로 실시하게 되었다. 병원간호사는 1983년부터 이미 서비스 능력교육인 전문직 보수교육을 받아오고 있었으며, 1990년대부터는 의료기관의 경영전략과 맞물려 서비스 성향교육이 추가되었고, 최근에는 간호서비스의 품질 향상을 위한 전략의 하나로 더욱 강화되고 있는 실정이다. 반면에 생명윤리나 전문직업인으로서의 윤리교육은 매우 미미하여, 병원간호사들이 환자들의 "자율성과 권리가 침해당하지 않도록 돕는 권리 옹호자이자 대리자"로서 역할을 수행하는데 장애가 되고 있다. 생명윤리는 현대의 과학 기술이 초래한 여러 문제들에 맞서서 인간의 생명과 인격의 자유와 존엄에 대한 위험이 내재하는 학문과 연구 분야라면 어디서나 요구되는 것이다. 특히 병원간호사들은 간호현장에서 '죽음'과 관련되어 많은 생명윤리적 문제에 직면하고 있다. 사회적 쟁점으로 대두되고 있는 생명윤리적 문제들의 요소들과 생명윤리적 논거들에 대하여 병원간호사들은 올바르게 인식할 필요가 있다. 아울러 환자라는 한 인격체의 전체적 균형과 조화를 존중하고 배려하는 동시에, 신체적 차원에서의 간호만 아니라 영적 차원에서의 돌봄에도 초점을 맞추는 '전인 돌봄'을 지향하는 간호야말로 오늘의 의료현장에서 일어나는 생명윤리문제를 해결할 수 있는 가장 중요한 인적 요소라 하겠다. 이를 위해서는 첫째, 병원간호사에 대한 생명윤리 교육이 간호대학의 교육과정에서부터 시작되어야 한다. 간호윤리 교육에 기본적으로 생명윤리가 포함되지만, 생애 주기별로도 생명윤리 교육을 접목시켜야 한다. 둘째, 병원현장에서의 생명윤리 교육이 사례중심의 보수교육으로 정기적으로 시행되어야 한다. 셋째, 병원간호사들이 환자와 가족을 대상으로 죽음에 대하여 적절한 간호중재를 할 수 있는 능력을 키우기 위한 교육프로그램이 필요하다.

영문 초록

Nowadays, changes in health care environments have made hospitals to adjust business strategy in order to improve competitiveness and to accomplish customer satisfaction. This has led most hospitals offer service education program to their employees including nurses. Hospital nurses already have been taking continuing education programs to improve their qualities of nursing service since 1983. By the 1990s, hospitals have started to add some kinds of service education courses for customers to their in-service programs for nurses according to business strategy. Now, service education has become an important strategy for a higher level of health care service. However, nurses often tend to fail to play their roles as 'a representative and a supporter who can help protect patient's autonomy and right' due to the lack of education on bioethics and professional ethics. Bioethics should be considered in any field where human life, free expression of personality, and human dignity might be threatened by modern scientific technology. Since hospital nurses face a large number of bioethical problems that are related to 'death' while on duty, they should have correct understanding regarding bioethical issues: the criteria or affecting factors to make a decision such as withdrawing life-sustaining treatment or euthanasia. In addition, nurses should be able to respect and take a good care of patient both physically and spiritually. By doing this, nurses will be able to accomplish 'holistic care', and 'holistic care' will be the key factor to solve bioethical problems rising in health care fields. In conclusion, to build the nurse's proper attitude toward bioethical issues, following three aspects should be considered; First, bioethical education class should be offered in the curriculum of nursing colleges. Second, bioethical education based on clinical cases should be offered as a continuing educational program. Third, it is necessary to develop the education program which nurses can enhance their abilities to take care of patients and their families who are facing death.
