회복기 의료기관에서 퇴원하는 뇌졸중 환자의 인지기능과 신체기능 수준에 대한 연구

국문 초록

목적 : 본 연구에서는 서울시에 소재한 회복기 재활의료기관의 가정복귀율을 알아보고 가정복귀한 그룹과 타병원으로 전원한 그룹의 인지적, 신체적 기능 수준을 비교 분석하여 그 차이를 알아보고자 한다. 연구방법 : 서울 소재의 한 회복기 의료기관에서 2019년 4월부터 8월까지 퇴원한 환자 총 213명을 대상으로 가정복귀율을 조사하고 가정으로 복귀한 군과 그렇지 않은 군의 인지적, 신체적 평가 결과를 통하여 그룹의 차이를 비교 분석하였다. 결과 : 서울 소재의 한 회복기 의료기관의 가정복귀율은 약 41%정도로 나타났으며 타기관으로 전원한 그룹이 가정복귀 그룹에 비해 발병기간이 유의한 수준으로 오래되었다. 발병 원인은 가정으로 복귀한 그룹이 그렇지 않은 그룹에 비해 뇌경색이 뇌출혈보다 많았으며 발병 부위는 유의한 차이가 나지 않았다. 두 그룹 간 재원일수는 유의한 차이가 나지 않았으며 가정으로 복귀한 그룹이 그렇지 않은 그룹에 비해 K-MMSE, K-MBI, BBS의 점수가 통계학적으로 유의한 수준으로 높게 나타났다. 결론 : 뇌졸중 환자들이 가정으로 복귀하기 위해서는 인지적 기능과 기본적 일상생활 수행능력 그리고 균형 감각 기능이 좋을수록 유리하기에 회복기 재활의료기관에서는 이 부분에 대한 집중적인 훈련이 필요할 것이다.

영문 초록

Objective : this study aimed to identify the factors that positively affect home return by examining its at a rehabilitation medical institution located in Seoul, and comparing the cognitive and physical functions of the returning home group to those of other hospitals. Methods : A total of 213 patients discharged from a recovery medical institution in Seoul from April to August 2019 were surveyed on their return home. Using a comparative analysis, the factors were assessed through the cognitive and physical evaluation results of those who returned home and those who did not. Results : At the recovery medical institution, the rate of returning home was about 41%, and the onset date were set significantly earlier than for group returning home from other hospitals. There was no significant difference in hospitalization duration between the two groups, and the K-MMSE, K-MBI, FMA, and BBS scores were statistically significantly higher than those of the group returning home. Conclusion : For stroke patients to return home, the better the cognitive function, basic daily performance ability, and balance sense function, the better the results. Based on these studies’ results, the rehabilitation medical system of the recuperative medical institution, which is closely connected to the community, is built to help patients. It is proposed that various studies should be conducted to improve the efficiency of returns to the home.
