탈북 청소년의 정신건강에 대한 문헌고찰

국문 초록

남한에 정착한 탈북민 중 10~29세에 해당하는 인원은 2000년대 이후 급격히 증가하여 전체 북한이탈주민의 40%에 육박하며, 탈북 과정에서의 외상, 남한사회에서의 차별 및 적응문제와 더불어 이러한 혼란 속에서 청소년기의 기본 발달과업을 완수해야 하는 이중부담을 가지고 있어 정신건강 문제에 특히 취약한 계층으로 생각된다. 이에 본 연구는 탈북 청소년의 정신건강 문제에 대한 해외 및 국내 연구를 개관하여 1) 탈북 청소년의 내재화 및 외현화 문제의 심각도와 2) 정신건강 문제 관련 위험 및 보호요인을 밝히 고자 하였다. 온라인 데이터베이스를 통해 탈북 청소년의 정신건강 관련 문헌 364편이 추출되었고 최종적으로 28편(국내 15편, 해외 13편)이 선정되었다. 그 결과, 비행/일탈, 공격성과 같은 외재화 문제는 남한 가정 청소년과 유사한 수준인 반면, 내재화 문제(예, 우울, 불안, 외상 후 스트레스 장애, 자살 위험성) 및 기타 위험행동(예, 음주, 흡연, 약물 사용)은 남한 가정 청소년에 비해 현저하게 높은 수준이었다. 더불어 부모와의 동반 입국 및 동거, 가족 및 남한친구와의 관계, 문화적응 스트레스, 부정적 자동적 사고 및 회복 탄력성과 같은 다양한 요인들이 내재화 및 위험행동 문제에 대한 위험/보호요인으로 연구되고 있었으며, 특히 외상 후 스트레스 장애의 여부, 부정적 자동적 사고 등 일부 요인이 탈북 청소년의 외상경험과 내재화 및 행동문제를 매개하는 것으로 나타났다. 연구결 과에 근거하여 탈북 청소년의 정신건강 문제의 예방 및 치료적 개입 전략을 위한 시사점과 본 연구의 제한점 및 그에 따른 추후 연구 방향을 제시하였다.

영문 초록

North Korean refugee youths (NKRY) in South Korea have been rapidly increasing and are at greater risk of mental health problems due to multiple adversities during and after the flight and their unique social status in South Korea. This review study addressed a range of psychological and social risks for mental health of NKRY. The objectives of this study were 1) to investigate the severity of internalizing problems and externalizing problems as well as other health risk behaviors in NKRY compared with native-born youths, and 2) to identify relevant risk and protective factors, from internal (i.e., personal) to external (i.e., environmental). To this end, a total of 28 empirical studies (Korean 15, International 13) on NKRY’s mental health, which were searched via online databases, were reviewed. As a result, internalizing problems (e.g., depression, anxiety, post-traumatic stress disorder, and suicidality) and risk behaviors (e.g., alcohol use, smoking, and drug use) were significantly prevalent in NKRY than in native-born youths. Multiple common risk/ protective factors, including family composition and relationship quality, peer relations, and acculturation stress, were noted. A few mechanisms by which these factors would exacerbate or mitigate the psychological/behavioral problems were also presented. The empirical evidence supports the heightened risk of internalizing problems and health risk behaviors in NKRY. Implications for prevention, intervention strategies for NKRY and suggestions for future studies are proposed.
