한국의료패널 자료를 이용한 제왕절개분만 결정에 미치는 영향

국문 초록

본 연구는 한국의료패널 자료를 활용하여 2013년 분만 이력이 있는 조사 대상 156명을 분석하였다. 대상자의 62.8%가 정상분만을 하였고, 37.2%가 제왕절개분만을 시행하였다. 30대가 78.2%로 가장 많았고, 35세 이상의 고령산모도 65.1% 이었다. 제왕절개분만에 관한 특성은 기왕의 제왕절개에 의한 반복적 제왕절개분만이 31.0%, 정상분만 진행의 실패가 20.7%, 태아 위치 이상이 12.1% 순 이였다. 그러나 진통에 대한 불안 등의 이유로 산모 자신의 선택에 의한 제왕절개분만도 20.6% 이었다. 임신성 고혈압을 가졌거나, 임신기간이 37주 미만인 경우 100% 제왕절개분만이 시행되었다. 분만 후 51.9%가 산후 조리 시설을 이용하였고, 지출비용은 100만원 미만이 66.7%로 가장 많았다. 따라서 임신부의 건강상태에 따라 분만 행태가 결정된다는 것을 알 수 있었고, 제왕절개율을 감소시키기 위하여 이러한 요소들을 해결할 수 있는 교육프로그램 개발 및 정부 제도적 개선이 필요한 것으로 사료된다.

영문 초록

In this study, 156 persons making a delivery in 2013 were analyzed using the data from the Korea Health Panel. 62.6% of the subjects made a normal delivery and 37.2% made a Caesarean delivery. Women in their thirties formed 78.2% and those aged 35 or older formed 65.1%. The most frequent characteristic of Caesarean deliveries was repeated Caesarean deliveries due to the prior Caesarean section (31.0%), followed by failure in normal delivery (20.7%) and abnormal position of the fetus (12.1%). 20.6% were Caesarean deliveries on mothers' own choice due to anxiety about labor. 100% of those who had pregnancy-induced hypertension or who were in the 37weeks of pregnancy made a Caesarean delivery. After the delivery, 51.9% used a post-natal care facility and 66.7% spent less than 1 million won in post-natal care. On the basis of the results, delivery type depended on mothers' health status. To reduce the rate of Caesarean deliveries, therefore, it is necessary to develop education programs that can solve these problems and to improve governmental institutions.
