요척관절의 편위에 영향을 미치는 요인에 대한 임상적 분석

국문 초록

[서론] 이 연구는 요척수근 관절에서의 비정상적 편위를 일으키는 요인을 찾는데 목적이 있다. [방법] 본 연구를 위한 피험자는 근골격계가 정상적으로 발달한 건강한 성인으로 19세에서 64세까지의 남녀 90명을 대상으로 하였으며 특수 고안한 각도기를 이용하여 요척골의 각각의 최대편위와 총 편위를 산출하였다. 각도기는 편평한 테이블(30cm×45cm)에 고정시키고 전완을 회내 상태로 하여 요척관절의 유연성을 측정하였다. 편위를 측정하기 위한 움직임의 중심은 중지와 3번째 중수골의 중심을 통과하는 가상의 축과 요골 및 척골의 경상돌기 중앙이 만나는 점으로 정했다. 이 움직임의 중심을 각도기의 회전의 중심에 맞추고 중지의 하단에는 플라스틱 마커를 끼워 각도기의 영점과 일치시켰다. 피험자는 팔꿈치를 편안히 이완시킨 상태에서 움직임의 중심을 통해 편위를 시도하였으며 마커가 척골 쪽으로 최대한 이동했을 때의 편위각을 측정하였다. 편위시에는 능동적으로 움직인 최대값을 측정하여 기록하였으며 요척골 편위는 양손을 모두 두 번씩 측정하여 평균값을 기록하였다. 연령, 성별, 노동 및 운동경험, 골절 및 염증경험, 그리고 주사용손 등의 5개 독립변인에 대한 상관분석 및 t-test를 실시하였다. 통계분석은 PASW V18.0 통계 프로그램을 사용하였으며 유의수준은 p<.05로 하였다. [결과] 각 독립변인에 대한 요골 및 척골편위는 연령을 제외하고는 모두 유의하지 않은 것으로 나타났다. 또한 각 독립변인에 대한 요골 및 척골의 총 편위는 오른손 총 편위가 주사용손과 부사용손 간에 유의한 차이가 있는 것으로 나타났다. 연령의 경우는 오른손 척골편위와 오른손 총 편위가 유의하게 나타났다. 연령은 주사용손이나 부사용손의 총 편위와 유의한 상호효과가 있는 것으로 밝혀졌다. 노동 및 운동경험에 있어서는 주사용손과 부사용손 모두 경험 유무간에 유의한 차이가 있는 것으로 나타났다. 골절 및 염증경험에 있어서는 주사용손의 경우, 경험 유무간에 유의한 차이가 있는 것으로 나타났다 [결론] 노동 및 운동경험이 없는 경우 주, 부사용손의 총 편위가 유의하게 크게 나타났고 골절 및 염증경험이 없는 경우 주사용손의 총 편위가 유의하게 크게 나타났다.

영문 초록

[INTRODUCTION] The purpose of this study was to investigate the factors causing abnormal deviation of radioulnar carpal joint. [METHOD] Subjects of this research were 90 men and women ranging 19-64 years of adults with normally developed musculoskeletal system. Maximal deviation of each radial and ulnar deviation, and total deviation were produced using a specially designed goniometer. The goniometer was stabilized on the flat table and flexibility of radioulnar joint was measured with forearm pronated. The center of the movement for deviation was determined by the cross-point made by the imaginary axis which passes the center of middle finger and 3rd metacarpal bone, and the halfway point of styloid processes of the radius and the ulna. The center of movement was positioned on the center of goniometer and plastic marker was inserted on the distal pha-lange of the middle finger and matched with the zero point of the rotation. The subjects relaxed the elbow and tried deviation thru the center of the movement, and the angle of maximum deviation which actively moved was recorded. Each radioulnar deviation was measured twice and the average was recorded. Correlation analysis and t-test were performed for the analysis of 5 independent variables such as age, sex, experience of labor and exercise, experience of fracture and inflammation on the wrist, and major hand. PASW V18.0 statistic program was performed for the analysis and the significance level was p<.05. [RESULTS] The radial and ulnar deviations on each of the independent variables were found to be insignificant except age. Also, total deviation of radius and ulna for the independent variables was found to be significant between major and minor hand. Ulnar deviation and total deviation of right hand were found significant for age and total deviations of major and minor hand were found to have significant interaction effect with age. For major and minor hand, there was a significant difference between having experience and no experience of labor and exercise. For major hand, there was a significant difference between having experience and no experience of fracture and inflammation. [CONCLUSION] Total deviation of minor hand had sig-nificantly greater interaction effect with the no experience of labor and exercise. Total deviation of major hand had significantly greater interaction effect with no experience of fracture and inflammation of wrist.
