이상두위를 보이는 눈떨림 환자의 수술 후 장기간 임상 양상 및 재발에 관여하는 인자

국문 초록

목적: 이상두위를 보이는 눈떨림 환자에서 장기간 임상 양상을 관찰하고 수술 후 재발에 영향을 미치는 요인을 알아보고자 한다. 대상과 방법: 1985년 1월부터 2013년 4월까지 고려대학교 안암병원에서 이상두위를 동반한 눈떨림으로 수평근의 modified Kestenbaum수술을 시행 받고 2년 이상 추적관찰이 가능했던 46명의 의무기록을 후향적으로 분석하여 환자들의 수술 전후 logMAR 시력, 이상두위정도를 통해 수술 효과를 평가하였다. 최종내원 시 머리돌림이 10° 이상이거나 재수술을 받은 경우를 재발로 정의하고, 재발군과 성공군,재수술군과 비재수술군으로 나누어 이상두위의 재발과 재수술에 미치는 요인을 분석하였다. 결과: 시력은 술전 좋은 눈 0.38, 나쁜 눈 0.42에서 술후 최종내원 시(평균 124개월 후) 각각 0.15, 0.21로 유의하게 호전되었다(각p＜0.001). 머리돌림은 술전 41.41°, 최종내원 시 2.61°로 술후 유의하게 감소하였다(p＜0.001). 재발률은 23.9%, 재수술률은 10.9%였고 지능저하, 약시, 술전 시력, 술전 사시, 술전 머리돌림 정도는 재발 및 재수술 여부와 관계가 없었다. 첫 수술 연령은 재수술군이 유의하게 낮았고(p=0.009) 술후 6개월째 좋은 눈, 1년째 좋은 눈과 나쁜 눈 시력은 재수술군이 유의하게 불량하였다(각 p=0.034,0.012, 0.008). 결론: 이상두위를 보이는 눈떨림 환자에서 수술 후 두위와 시력이 호전된 것으로 나타났다. 첫 수술을 받은 나이가 어리거나 술후 시력이 불량한 경우 재수술률이 높은 경향을 보였으며 이상두위 재발에 대한 향후 추가적인 전향적 연구가 필요할 것으로 생각한다

영문 초록

Purpose: To evaluate the long-term outcome and recurrence of abnormal head posture after modified Kestenbaum surgery in patients with nystagmus. Methods: The medical records of 46 patients who underwent modified Kestenbaum procedure in horizontal recti muscles for nystagmus with abnormal head turn were retrospectively reviewed. We assessed the effect of surgery by comparing preoperative and postoperative clinical data such as visual acuity (log MAR) and degree of head turn. A reoperation or abnormal head turn of 10° or more at final visit were defined as recurrence. Patients were divided into 2 groups according to the presence of recurrence or reoperation. Clinical factors associated with recurrence or reoperation were compared between the 2 groups. Results: The mean visual acuity was 0.38 in the better eye and 0.42 in the worse eye before surgery, which improved to 0.15 and 0.21 after surgery (each p < 0.001), with a mean follow-up period of 124 months. The mean degree of head turn was 41.41° preoperatively and was changed to 2.61° postoperatively (p < 0.001). The recurrence rate was 23.9% and the reoperation rate was 10.9%. Mental retardation, amblyopia, preoperative visual acuity, degree of head turn, and presence of strabismus were not associated with recurrence or reoperation. The mean age at first surgery was significantly lower in the reoperation group (p =0.009). The mean visual acuity in the better eye at postoperative 6 months and in better and worse eyes at postoperative 1 year was significantly worse in the reoperation group (p = 0.034, 0.012 and 0.009, respectively). Conclusions: The visual acuity and head turn was improved after modified Kestenbaum surgery in patients with nystagmus and abnormal head posture. The reoperation rates were associated with earlier age of first operation and worse postoperative visual acuity. However, further prospective studies are necessary to clarify these factors.
