안허혈질환에서 경동맥초음파로 측정된 내중막두께와 혈중지질검사의 이상소견

국문 초록

목적: 안허혈질환에서 경동맥초음파소견과 혈액학적 이상소견을 알아보고 심혈관계 질환의 위험도 및 연관관계를 알아보고자 한다. 대상과 방법: 2010년 1월부터 2013년 6월까지 본원에서 앞허혈시신경병증(anterior ischemic optic neuropathy [AION], n=45), 중심정맥폐쇄(central retinal vein occlusions [CRVO], n=34), 중심동맥폐쇄(central retinal artery occlusions [CRAO], n=23), 안허혈증후군(ocular ischemic syndrome [OIS], n=27)을 진단 받은 환자를 대상으로 경동맥 초음파와 혈액검사를 시행하여 내중막두께(intima-media thickness, IMT), 혈관협착(stenosis)의 유무를 측정하고 혈액검사를 시행하여 건강한 대조군(n=107)과 비교하였다. 결과: 평균 나이는 전체 환자군에서 60.0 ± 14.1세였으며, 대조군은 56.1 ± 9.1세였다. 동측 경동맥 내중막두께는 대조군과 비교하여 OIS와 CRAO 군에서 유의하게 증가되어 있었다(각각 p＜0.01, ＜0.01). 경동맥의 협착은 OIS 군에서 17명(62.9%)으로 가장 높았으며, CRAO 군에서 5명(21.7%), CRVO 군에서 2명(5.9%), AION 군에서 1명(2.2%) 순으로 나타났다. 대조군에서는 의미 있는 협착은 관찰되지 않았다. 총 콜레스테롤 수치는 환자군과 대조군에서 유의한 차이를 보이지 않았으나, 저밀도 지단백 콜레스테롤 수치는 모든 환자군에서 대조군보다 유의하게 높았다(각각 p＜0.01, ＜0.01, ＜0.05, ＜0.01). 결론: 안허혈질환이 있는 환자들은 경동맥 질환 및 이상지질혈증이 선행하였을 가능성이 있어 경동맥 초음파와 혈중 지질검사 등의 전신적인 검사가 필요할 것으로 생각되며 심혈관계 질환의 위험요소에 대한 평가와 관리가 필요하다.

영문 초록

Purpose: To analyze the findings of carotid Doppler and lipid profiles in patients with ischemic ocular disease and as well as the risk and correlation between cardiovascular and ischemic ocular diseases. Methods: We analyzed the intima-media thickness (IMT) and existence of vascular stenosis using carotid Doppler and blood lip-id profiles of the study patients diagnosed with anterior ischemic optic neuropathy (AION group, n = 45), central retinal vein oc-clusion (CRVO group, n = 34), central retinal artery occlusion (CRAO group, n = 23), or ocular ischemic syndrome (OIS group, n = 27). The study patients who visited Chonbuk National University Hospital from January, 2010 to June, 2013 were compared with healthy control (control group, n = 107). Results: The average age of the study patients was 60.0 ± 14.1 years and 56.1 ± 9.1 years for the controls. Ipsilateral common carotid artery-IMT was thicker in the OIS and CRAO groups than the control group (p < 0.01 and <0.01, respectively). The num-ber of patients with carotid stenosis was highest in the OIS group (n = 17, 62.9%) followed by the CRAO group (n = 5, 21.7%), CRVO group (n = 2, 5.9%), and AION group (n = 1, 2.2%). No significant stenosis was found in the control group. Total blood cholesterol was not significantly different between the study patients and controls. Low-density lipoprotein cholesterol was high-er in every patient group than the control group (p < 0.01, <0.01, <0.05 and <0.01, respectively). Conclusions: The ischemic ocular disease likely precedes carotid artery disease and dyslipidemia. Therefore, general examina-tions such as carotid Doppler, lipid profile and risk factor evaluations are needed in patients with ischemic ocular disease.
