우울이 연소·고령 노인집단의 건강증진행동에 미치는 영향

국문 초록

본 연구는 우울이 노인의 건강증진행동에 미치는 영향이 연소노인과 고령노인 간에어떻게 다르게 나타나는지를 검증하고자 하였다. 서울 및 경기지역 5개 복지기관에서65세 이상 노인을 대상으로 한 면접 설문조사 결과 얻어진 2차 자료를 사용하였고, 총372명이 분석대상에 포함되었다. 위계적 회귀분석 결과 첫째, 우울은 연소노인집단과 고령노인집단의 건강증진행동에 통계적으로 유의미한 부정적 영향을 미쳤으나 우울의 상대적 영향력은 연소노인집단에서 더 크게 나타났다. 둘째, 다른 변수들에서도 연소노인과 고령노인 간 차이가 나타났다. 연소노인의 경우에는 성별, 스마트폰 사용 여부, 소득, 사회적 관계 만족도, 건강 관련 사회적 지지가 고령노인의 경우에는 주관적 건강상태, 스마트폰 사용 여부, 사회적 관계만족도, 건강 관련 사회적 지지가 건강증진행동에 통계적으로 유의미한 영향을 미치는 것으로 나타났다. 연구결과를 토대로 연소노인의 우울조기스크리닝과 우울에 대한 임상적 개입, 고령노인에 대한 차별화된 경제적 지원과 건강증진서비스 등의 정책적, 실천적 접근에 대한 함의가 논의되었다.

영문 초록

This study examined the effects of depression on the health-promoting behavior among young-old and old-old. In-person interviews using questionnaires were conducted to the elderly aged 65 and over at 5 community welfare centers, Seoul and Gyronggi Province. A total of 372 participants were included in the analysis. The results of hierarchical regression analysis were as follows. First, depression was a strong predictor of the health-promoting behavior both in young-old and old-old, but the effect was stronger in old-old. Second, the effects of other control variables were different depending on the age group. Sex, smartphone-use, income, the satisfaction in social relationship and health-related social support significantly predicted the increase of health-promoting behavior in young-old. On the other hand, subjective health status, smartphone use, the satisfaction in social relationship, medicaid status and health–related social support were strong predictors in old-old. Based on the study results, differentiated policy and practice implications were discussed including early screening and clinical intervention strategy of depression for young-old and economic support and health promoting service for old-old.
