초발정신분열병 환자들의 퇴원후 치료적 순응

국문 초록

목 적： 본 연구에서는 초발정신분열병 환자의 사회인구학적 특성과 임상적 특성을 조사하여 초발정신분열병 환자의 퇴원 후 치료적 순응도와 연관관계를 비교하고자 하였다. 방 법： 2003년 1월 1일부터 2007년 12월 31일까지 조선대학교병원 정신과에서 입원 치료 후 퇴원한 초발정신분열병 환자 74명을 각각 치료유지군과 치료중단군으로 분류하였고 의무기록 확인을 통하여 사회인구학적 특성과 임상적 특성을 조사하였다. 결 과： 치료유지군 환자의 GAF점수는 치료중단군 환자의 GAF점수보다 높은 점수를 보였다. 또한 치료유지군 환자의 퇴원형태는 권고퇴원의 비중이 높았지만 치료중단군 환자의 퇴원형태는 거역퇴원의 비중이 더 높았다. 이 두 가지 요인은 통계적으로 의미 있는 수준으로 각 환자군 사이에 차이가 있었다. 결 론： 퇴원시 GAF척도와 퇴원형태는 초발정신분열병 환자의 퇴원 후 치료순응도와 상관관계가 있는 것으로 생각된다.

영문 초록

Objectives：This study aimed to investigate socio-demographic and clinical characteristics of first episode schizo-phrenia to improved therapeutic compliance after discharge from a psychiatric hospital in patients with first episode schizophrenia. Methods：After grouping of follow-up and drop-out patients through chart review with 74 first episode schizo-phrenia patients admitted at department of psychiatry of Chosun university hospital from Jan. 1 2003 to Dec. 31 2007, differences between the two groups were investigated about socio-demographic and clinical characteristic. Results：Follow-up patients’ global assessment functioning score was higher than drop-out patients’ global as-sessment functioning score. Follow-up patients had recommend advised discharge mode, but drop-out patients had against discharge mode. There were significant differences in factors of global assessment functioning score(p=.002) and mode of discharge(p=.043) between the both group. Conclusions：Clinicians should be concerned for global assessment functioning score and mode of discharge to improve compliance after discharge.
