망막분지정맥폐쇄환자에서 트리암시놀론의 유리체내 주입술과 후테논낭하 주입술의 비교

국문 초록

목적: 황반부종을 동반한 망막분지정맥폐쇄 환자에서 트리암시놀론의 유리체강내 주입술과 후테논낭하 주입술의 임상효과를 비교하고자 하였다. 대상과 방법: 황반부종을 동반한 망막분지정맥폐쇄 환자 25명/25안을 두 군으로 나누어 13명/13안에서는 유리체강내 트리암시놀론을 주입하였고 다른 12명/12안에서는 후테논낭하 트리암시놀론을 주입한 뒤 두 군의 중심망막두께와 시력, 안압을 측정하였으며, 시술 전, 시술 후 1달, 3개월, 그리고 6개월에 측정하여 비교하였다. 결과: 유리체강내 주입군은 시술 후 1개월부터, 후테논낭하 주입군은 시술 후 3개월부터 유의한 중심황반두께의 감소와 시력호전을 보였으며 이후 악화되지 않고 유지되었다. 두 군간 비교에서는 시술 후 1달째 유리체강내 주입군에서 중심황반두께의 감소와 시력호전이 유의하게 높았다. 또한 시술 후 1주일, 1달째 유리체강내 주입군에서 안압상승율이 유의하게 높았다. 결론: 유리체강내 스테로이드 주입술이 단기간 황반부종의 치료에 보다 효과적이나 안압상승의 위험이 더 크고, 장기적으로는 테논낭하 주입술도 망막분지정맥폐쇄의 황반부종을 치료할 수 있는 안전하고 효과적인 방법으로 생각된다.

영문 초록

Purpose: To anterospectively evaluate the clinical results of intravitreal (IVT) and posterior subtenon (PSTT) injection of triamcinolone acetonide (TA) for macular edema with branched retinal vein obstruction (BRVO). Methods: Twenty five eyes of 25 patients with macular edema of BRVO was assigned two groups. 13 eyes in IVT group received intravitreal TA injections (4 mg/0.1 ml) and 12 eyes in PSTT group received subtenon TA injections (20 mg/0.5 ml). Central Macular Thickness, visual acuity, and intraocular pressure were measured at before and 1, 3, 6 month postoperatively. Results: The mean CMT was significantly dereased at 1 month postoperatively in IVT group, and at 3 months postoperatively in PSTT group. And the recuction was maintained until 6 month postoperatively in both groups. The mean CMT in IVT group was significantly more decreased than PSTT group at 1 month postoperatively. Visual acuity in IVT group was significantly more improved than PSTT group at 1 month postoperatively. And there is no worseness of CMT and visual acuity in both group. The percentage increase of IOP was significantly higher in IVT group than PSTT group in 1 week, 1 month postoperatively. Conclusions: IVT injection seems to have more rapid action on BRVO-associated macular edema than PSTT injection. But IVT injection has more risk of IOP increase. PSTT injection also seems to be a safe and effective injection for BRVO-associated macular edema.
